We are committed to providing a responsive service that recognises the needs of those patients who may be disadvantaged in accessing care and treatment due to disability or other health inequalities.
How we can help you
We have Learning Disability Liaison Nurses at the Diana, Princess of Wales Hospital, Grimsby and at Scunthorpe General Hospital. Our working hours are Monday to Friday, 8.30am to 4.30pm.
We help support people with learning disabilities and autism, and their families and carers across the hospitals. We work clinically with patients every day, as well as ensuring that staff are given advice and support so that reasonable adjustments can be made to patient care. We play a critical role in raising the profile and status of people with learning disabilities and autism when attending a hospital.
Our nurses can help with:
- Communication advice.
- Pre-admission and discharge planning.
- Desensitisation planning.
- Carer support and advice.
- Facilitation of any patient appointment.
- Accessible information development.
- Advice and support or reasonable adjustments.
